I thought for sure I would need a X-Large. I have about a 36 inch waist, but wear an XL Cheapo Xcel 5/4/3. I tried a Pyro at Gorge Performance, and would have gone with the XL if the sales guy hadn't made me try the Large. As it is, I think the Large has plenty of room and I needed to tighten it down.. You definitely don't want it too big.

I think a lot of harnesses run 31-33 or 32-34 waists for the medium. I'm a 32 and wear mediums (although not dakine) and they're roomy even over a 4/3. I'd recommend trying one if you can. I bet a medium would work. Having too big a waist harness can suck because it'll slide up and be uncomfortable so you wanna make sure you get one that you can tighten down real nice.

I'm a 33 and wear a Large Dakine Pyro over a 5/4. Fits great. Tried the medium...pretty tight. Extraction is friendly to frozen fingers during winter.

I've got a ~32" waist and use a medium Pyro and have no problems getting it over wetsuits. I do struggle to squeeze it over winter garb but it has plenty of room for adjustments. Those straps are a bitch to move though... which is a good thing most of the time.
I had a L Fusion and swam in it (figuratively, and literally on occasion.)

34 and wear a L DaKine Renegade. XL is too big. I can use the M but it's basically too small.

36" and wear the Dakine Pyro L. Works great with board shorts, but also adjusts out enough to wear with wetsuits, drysuits, or over outer clothes when snowkiting. I don't think you have to upsize based on what you would typically wear. There is a lot of adjustment in these harnesses. I do use a spreader bar pad.
